You fill out this form and send in some paperwork. Then a nurse will reach out to determine how many weekly hours of care you can get with CDPAP (Consumer Directed Personal Assistance Program).

Choose a Caregiver

You choose a caregiver you love: your daughter, son, relative, or friend. We schedule an orientation call to finalize your enrollment.
